This was a brand spanking new Airbus 340-600, one of the largest airplanes ever built. It crashed without a single hour of airtime.

The flight crew of Abu Dhabi Aircraft Technologies was conducting some pre-delivery tests on the ground. to make a long stroy short, the crew took all 4 engines to full power, making the aircraft think it was taking off. A safety feature kicked in and ended up crashing the aircraft. This is a $220 million loss.
